Initial Consultation

Slow Medicine is a philosophy of exploring health with a trained investigator. How to find balance with you? Learn the patterns of your symptoms to identify the adjustment. Dr. Jill Ghormley charges a small fee for the initial consultation and the visit is focused on applying slow medicine to your life and feeling the best.

Preparing For Your First Visit

New patient paperwork, including the patient information form, history form, and patient responsibility form, require 10-15 minutes to fill out before your first visit. Our staff is happy to email these forms in advance of your visit date for your convenience.

Any relevant medical records are best brought with you at your first visit. A release of records form is available from our office to release records from other health care providers. The form is ideally filled out in advance of or after your first office visit. The process to receive medical records can take up to 2-3 weeks.

Billing and Insurance

Hands on Natural Medicine does not accept insurance. You will be expected to pay in full at the time of your visit. After your initial evaluation with Dr. Ghormley, you will be provided with a treatment plan which will include the total cost for all procedures discussed before we schedule you for your appointment. For those wishing to use insurance please let us know and we are happy to provide you with a superbill that you can submit to your insurance company for reimbursement for the visit. We do not contract with or bill your insurance but many of our procedures are covered under insurance plans. If you wish to submit a superbill to your insurance, it is important you check your benefits before coming to the office. Please note that even if Dr. Ghormley is in your plan, there is no guarantee for coverage.

Medical Record Confidentiality

Your medical records are kept in the strictest of confidence. No individuals have access to your medical records without your prior written authorization.

Prescription Drugs and Refills

In order to expedite requests for prescription refills, please contact your pharmacy and not our office. The pharmacy will fax us a request, which provides us with a necessary paper trail to document the refill.

Emergencies

We do not provide on-call service as we are not a primary care clinic. Please call 911 for help or your doctor or Primary Care Provider.
